Central Toronto Youth Services (CTYS) is an accredited Children’s Mental Health Centre with a range of services and programs for youth and families across Toronto. These services are client-centered and flexible, geared for youth and families with a range of mental health needs who may have difficulty accessing traditional office-based services.

 

New Outlook:  Community Support Program at CTYS

 

The Community Support Program (CSP) is one of five (5) New Outlook programs funded by the Ministry of Health.  These programs provide mental health assessment, support and case management for transitional aged youth (aged 16-24) and their families, who experience difficulties mastering both the developmental and transitional tasks of adolescence and young adulthood.

Using strength-based recovery approaches to counselling and case management, members of the CSP program work with clients to improve their quality of life and instill hope.  Through the exploration of their experiences of mental health, staff incorporate wellness and recovery frameworks to build relationships to empower clients to access resources and opportunities.  The CSP staff also integrate interventions that are trauma-informed and evidence-based, and take into consideration ways to prioritize social justice and inclusion.  This role supports clients through a blended service model with both community outreach and virtual work.  A flexible work schedule is required to accommodate individual and family schedules in the evenings.  For more details, see www.CTYS.org

Primary Responsibilities:

·         Work collaboratively with youth, service providers and families/ caregivers, to develop comprehensive and effective service delivery plans for transitional aged youth (aged 16-24) utilizing a wellness and recovery framework

·         Provide brief and ongoing counselling and therapeutic services informed by trauma theory and evidence informed practices

·         Assist youth in identifying and acquiring skills needed to meet their goals

·         Facilitate access to resources such as psychiatry, housing, substance abuse programs, crisis response services, etc. and to provide crisis response/ safety planning, as necessary

·         Provide brief service and longer-term case management, as needed

·         Support advocacy and resource brokerage

·         As required, work with families from diverse cultures, utilizing interpreters as needed

·         Provide flexible and accessible service delivery including virtual or in-person service options, and some evening work

·         Participate in team, staff, and supervision meetings

·         Maintain all records and reports as required by the CTYS policies and procedures

·         Other duties as assigned

 

Qualifications:
·     Bachelors in Social Work or related health care discipline and/ or combination of equivalent education and experience

·     Masters level degree in a counselling related field is preferred

·     Registration in a regulated health college (e.g. R.P. /R.S.W.) is preferred

·     Identification with and/ or experience supporting racialized communities specifically Black/ African/ Caribbean diasporic communities is preferred

·     Minimum of three (3) years experience in a mental health setting

·     Knowledge of and commitment to practicing from a decolonizing, anti-racist and anti-oppressive framework

·     Experience working with youth and families from diverse cultural and socio-economic backgrounds

·     Demonstrated experience working from an anti-oppressive framework with youth and families from diverse ethno-cultural and socio-economic backgrounds including youth with diverse intellectual abilities and racialized communities

·     Experience with and understanding of youth with serious mental health challenges that impair daily functioning

·     Knowledge of appropriate therapeutic approaches and interventions

·     Demonstrated skills in assessment, formulation, counselling, and case planning and coordination

·     Ability to work independently and as part of a team

·     Excellent verbal and written communication skills

·     Familiarity with resources supportive to CTYS clients (e.g. 2SILGBTQ+ and BIPOC youth) and experience referring clients to community resources

·     Comfort and proficiency in using technology to engage youth and families (e.g. phone, text, and video) and in completing clinical documentation (e.g. Microsoft Word, Adobe, and Outlook)

·     Proficiency in French language is an asset

·     Valid Ontario driver’s license and access to a vehicle is an asset

·     Proof of Full Covid-19 vaccinations (2 doses) is required

·     Clear Vulnerable Sector Screening Check is required

Salary Range: $ 59,486.21 – $ 71,029.64

Youth Justice Mental Health ACCESS Program at CTYS

The Youth Justice Mental Health ACCESS Program provides mental health assessments and brief mental health service to youth and families referred from Toronto probation and mental health court workers.  This is an exciting opportunity to be a member of YJ Mental Health ACCESS Team, supporting youth and families involved with the youth justice system. The Family Focused Outreach Worker will focus specifically on the engagement of family to improve family functioning by affecting change in the family system.  Family focused intervention would be based on a community outreach model.  The Family Focused Outreach Worker would meet with the family in the community and/or family home with the possibility of virtual work if indicated by need.  Flexibility is required to work some evenings to accommodate family schedules.

Primary Responsibilities:

To engage families of youth identified and referred by Toronto probation services, mental health court workers and collaborative agency partners
Work collaboratively with youth and parents to develop individualized assessment and effective service delivery plans using strengths based family systems approaches, a trauma informed and Anti-Oppression lens
To work effectively with mental health collaborative partners, youth, parents, probation services and other workers who are part of the youth’s team
Engage in family counselling by working with parent(s) and/or young person jointly and individually to address such things as relational and communication issues within family, develop parenting skills and competency and provide psychoeducation for parents
Support parents to address individual and systemic issues in order to build stability and support for youth
Work with parents to address challenges and develop skills in systems navigation
To advocate for and transition youth and families to community-based services and other resources when required
To develop and implement parenting workshops for parents and/or service workers
Participate in program and evaluation development
Ensure that all reporting and documentation meets requirements and are completed in a timely manner
Participate in supervision, program meetings, all-staff meetings, clinical consultations, peer consultations, committee/partnership meetings and professional development as required
Support and sustain values of decolonization and racial justice as central to the work, and seek interventions that advance leadership of those most impacted by harmful systems
Ensure clinical services are delivered in a manner consistent with all applicable agency policies and procedures and accreditation standards
Qualifications:
Demonstrated experience working from an anti-oppressive framework with youth and families from diverse ethno-cultural and socio-economic backgrounds including youth with diverse intellectual abilities and racialized communities
Identification with and/ or experience supporting racialized communities specifically Black/ African/ Caribbean diasporic communities
Master’s degree in Social Work or related helping profession and/ or a combination of equivalent education and experience
Registration in a regulated health college (e.g. R.P. /R.S.W.)
Minimum three (3) years’ experience working in children’s mental health
Knowledge of and experience working in the Youth Justice system
Experience working with families from a community outreach model
Experience working with trauma informed, resiliency and strength-based approaches
Demonstrated ability to work collaboratively and effectively with the youth justice system and collateral service providers
Strong interpersonal skills and ability to build rapport and engage marginalized youth and their families
Experience running groups and facilitating workshops
Experience developing workshops and presentations
Knowledge of program development and evaluation is an asset

Youth Hostel Outreach Program at CTYS

The Youth Hostel Outreach Program (YHOP) is a relationship based mental health outreach program for young adults who are homeless/marginally housed and/or living in the youth shelter system.  The outreach model uses a relational approach to engage youth. After developing a trusting relationship, the worker will provide access to a mental health assessment as required and provide direct counselling support.

 

The successful candidate will directly address the complex and unique needs of young adults (16-24) with serious mental health challenges in youth shelters in Toronto by providing improved and coordinated access to treatment, accommodation and other services.  The position also requires the development of successful working relationships with the designated shelters to ensure client needs are met while providing consultation and education regarding mental health challenges.

 

The position will also provide specialized focused case management and mental health support and counselling from an outreach model for 16 and 17-year-old youth in the youth shelter system. When possible the worker will support relationship building with family to support rupture repairs.

 

Currently, services are provided both in-person and virtually.  Flexibility is required to work some evenings.

Primary Responsibilities:

To provide on-site, flexible advocacy services to transitional aged youth with serious mental health challenges in the shelter system
To work in close collaboration with hostel staff in identifying clients in significant need of services
To increase client access to resources, such as housing, treatment services, crisis response services, substance abuse programs, etc.
To provide ongoing support services to a small caseload and to provide brief service, as required
To provide support, education, information, and consultation to hostel staff
To identify and establish links with other support services and programs
To support and sustain values of decolonization and racial justice as central to the work, and seek interventions that advance leadership of those most impacted by harmful systems
To ensure clinical services are delivered in a manner consistent with all applicable agency policies and procedures, and accreditation standards
Qualifications:
Bachelor’s degree in Social Work (BSW) and/ or equivalent related degree and experience
Identification and experience supporting racialized and 2SILGBTQ+ communities
Experience and familiarity with the issues of homelessness and youth
Minimum three (3) years related experience working with youth with serious mental health and substance use challenges
Extensive knowledge of resources for the transitional age population
Experience providing virtual care is an asset
Commitment to practice from an Anti-Oppression, Equity, Anti-Black Racism (ABR) and Trauma Informed frameworks
Innovative and creative approach towards helping systems work collaboratively
Excellent assessment, formulation and case coordination skills
Demonstrated case management experience
Ability to work independently
Flexible work schedule including evening availability
